Lower limb for FRCS (Plast)

Lower limb and trauma covers soft tissue reconstruction of the leg and foot and the chronic wounds that bring patients to a plastic surgeon. Open fractures are managed by combined orthoplastic teams under BOAST and NICE standards, with Gustilo–Anderson classification, timing of debridement and definitive soft tissue cover, and the choice of local or free flap by the part of the leg involved. Pressure sores cover staging, the optimisation that must come before surgery, and flap reconstruction of sacral, ischial and trochanteric sores. Diabetic foot disease brings together vascular assessment, infection, offloading and amputation level. Lymphoedema covers diagnosis and staging, decongestive therapy, and surgical options from lymphaticovenular anastomosis and vascularised lymph node transfer to liposuction for fat-dominant disease.

What the paper tends to test

  • Expect open fracture questions built around the BOAST standard: antibiotics, debridement timing, combined orthoplastic planning and definitive cover.
  • The paper tends to test reconstruction by zone of the leg: what covers the proximal, middle and distal thirds and the foot.
  • Expect pressure sore questions that test optimisation — nutrition, spasticity, continence and offloading — before any flap.
  • Lymphoedema questions usually ask you to match the surgical option to the stage of disease and to whether the limb is fluid- or fat-dominant.

Classic traps and discriminators

  • BOAST open fractures: antibiotics ideally within 1 hour of injury; debridement immediately for highly contaminated wounds, within 12 hours for high-energy and within 24 hours for other open fractures; definitive cover within 72 hours if not achieved at debridement.
  • The Gustilo–Anderson grade is assigned after debridement in theatre, not from the wound’s appearance in the emergency department.
  • Gastrocnemius flaps cover the proximal third of the tibia and soleus the middle third; distal-third defects usually need a free flap.
  • Pressure sores are not reverse-staged as they heal, and a chronic wound that changes in character needs biopsy to exclude a Marjolin’s ulcer.

Two days after a nine-hour spinal operation, a 58-year-old man has a 6 cm area of intact, non-blanching, purple-maroon skin over the sacrum that is boggy and warmer than the surrounding skin.

A 32-year-old motorcyclist has an open tibial shaft fracture with a 6 cm anterior wound containing road grit and fragments of clothing. Intravenous antibiotics have been given, the limb is aligned and splinted, and the combined orthoplastic list starts in 4 hours.

Lower limb questions, answered

What are the BOAST timings for open fractures?

Intravenous antibiotics ideally within 1 hour of injury; debridement immediately for highly contaminated wounds, within 12 hours for high-energy and within 24 hours for other open fractures; and definitive soft tissue cover within 72 hours if not achieved at debridement.

Which flap covers a middle-third tibial defect?

A soleus flap is the classic local option for the middle third, with gastrocnemius for the proximal third. Distal-third and larger defects usually need free tissue transfer.

Is lymphoedema surgery examined in Section 1?

Lymphoedema is part of the lower limb curriculum. Expect questions that match lymphaticovenular anastomosis, vascularised lymph node transfer or liposuction to the stage and character of the disease.
